Sandhivata Explained: Ayurvedic Care for Osteoarthritis and Joint Pain

Knees that creak on the stairs, joints that stiffen after sitting, a dull ache that grows worse by evening. These are the everyday signs of osteoarthritis, the wear-and-tear condition that becomes more common as we age. Ayurveda calls it Sandhivata, literally Vata settled in the joints, and offers an approach built around lubrication, nourishment and protecting what remains.

A condition of dryness and depletion

Vata is dry, light and cold by nature. As we grow older, Vata naturally rises, and the cushioning within our joints begins to thin. The cartilage that once moved smoothly loses its lubrication, the surfaces grate against one another, and pain follows. Where rheumatoid arthritis is driven by toxins and inflammation, osteoarthritis is more a story of degeneration and dryness. That distinction shapes the whole course of treatment.

Restoring what dryness takes away

Because the root is dryness, the answer is nourishment and lubrication, both inside and out. This is why oleation therapies sit at the heart of Sandhivata care. Warm medicated oils are applied through full-body massage, or Abhyanga, to soothe the tissues and calm aggravated Vata.

For knee pain in particular, a treatment called Janu Basti is often used, where warm medicated oil is pooled over the knee within a dough ring and allowed to soak in. Patients frequently describe a noticeable easing of stiffness after a course of these sessions. Internal Basti therapy is also valued, since it addresses Vata at its main seat in the body.

Herbs and rebuilding strength

Ayurveda draws on herbs traditionally used to ease joint discomfort and support the tissues, often combined in classical formulations. Equally important are the rejuvenating Rasayana therapies, which aim to rebuild strength and slow further wear. Gentle, joint-friendly movement is encouraged, since complete rest tends to make stiff joints worse rather than better.

Everyday protection

Day to day, a few choices make a difference: staying warm, avoiding cold and dry foods that aggravate Vata, keeping to a comfortable weight to reduce load on the knees, and including warm oils in a regular self-care routine. Osteoarthritis cannot be reversed entirely, but a thoughtful Ayurvedic routine can ease the pain, improve mobility and help you stay active for longer.
