Naresh, a 40-year-old male from Telangana, first experienced back pain when he was around 18. Initially, the pain was mild, but over time it worsened, making it hard for him to carry out his daily activities. After undergoing an MRI at Manipal, he was diagnosed with a disc bulge. Notably, he had not suffered any accidents, but his mother also suffered from back pain, which might have contributed to his condition. For the past 10 years, his job has required him to sit for extended periods of up to eight hours daily, which may have contributed to the recurrence of his back pain specifically in January 2024. Even though the doctor recommended steroids, the pain persisted. For the last three months, he has been dealing with excruciating, sharp shooting pain that has made his walking difficult , forcing him into a posture that leans to one side and has severely disrupted his sleep. The doctor suggested either lifelong medication for pain management or surgery, but Naresh is hesitant about undergoing spine surgery at this age.He tried physiotherapy and other conservative treatments without much relief. He then sought help at Sree Subramania Ayurvedic Hospital.Currently he experiences radiating pain down his right leg, especially when walking even short distances. He Struggles to lie flat as he suffers from dull pain while sleeping and experiences pulling and shock-like sensations particularly when coughing or sneezing. His posture is affected by pain during movement, but he feels no pain when at rest and he has had difficulty getting up from a seated position

Lumbar spondylosis
Age related degeneration of the lower back vertebrae and discs is known as lumbar spondylosis. Osteoarthritis and degenerative disc disease are common names for this. Breaking down of one or more discs that separate the spine’s bones is a common sign of this disorder. Spondylosis usually hits the discs in the lower back area. When these discs bulge out, they can push against the spinal nerves that extend from the spinal cord to the rest of your body. This bulging can lead to nerve pain called sciatica, which travels along the sciatic nerve from your lower back down through your legs. As disc wears down, you may see bone spurs forming on the edges of the vertebrae. These bone spurs can also pinch the spinal nerves, causing weakness or numbness in your arms or legs. If they press on the spinal cord, you could face issues with walking and controlling your bladder or bowels. Eventually a disc can deteriorate completely, leaving no gap between two vertebrae, which can lead to limited movement, pain and nerve damage this pressure can cause numbness, paralysis and discomfort in your legs and back
What are the causes?
- Bad Posture: If one postures incorrectly while standing, sitting, or moving heavy objects, spondylosis may result from the tension on the lower back.
- Repetitive Stress: Over time, degenerative changes may result from repetitive stress inducing actions such lifting, twisting and bending that place causes a continuous strain on the lower back.
- Sedentary Lifestyle: Prolonged sitting or inactivity along with little exercise can weaken the back muscles and aggravate spondylosis.
- being overweight accelerates the deterioration of the lower spine by applying extra pressure to it.
- Spinal traumas: As we become old, previous spinal traumas can raise risk of developing spondylosis.
- 6. Age: Over time, deterioration happens
Symptoms of Lumbar Spondylosis?
- Low Back Pain: Persistent pain in lower back is the main symptom of lumbar spondylosis.
- Stiffness: Stiffness in the lower back mainly after periods of rest
- Clicking Sensation: From lower back while changing positions.
- Difficulty While Walking
- Pain Increases with Standing
- numbness, paralysis and discomfort in your legs and back
Diagnosis?
• By thorough assessment and clinical examination of patient
• X- Ray
• MRI /CT scan
