In cervical region (neck region) the commonest site is C5-C6, and occasionally C4-C5. These produce symptoms of shooting pain in the neck and along the upper limb. In the Lumbar region (low back) the commonest site is L5-S1, but L4-L5 disc or L2-L3 discs may also be affected. The pain may start from buttocks region and radiating towards lower limb, especially in thighs, knee and ankle. For some people pain will be constant in the low back region and also weakness will be felt in the lower extremities.

During clinical examination, the important tests we conduct is straight leg raise test (SLR) where the patient is made to lie down and lift the leg without bending the knee, which will produce pain that is due to the stretching of nerve root. In severe cases of disc prolapse, you will get cross leg SLR i.e. if the patient is having pain on the right leg, if you try to lift the left leg he will experience pain on the right side, which is an indication of severe disc prolapse.

In case of severe pain and not relieved for 2 to 3 weeks, further treatment should be started after undergoing specific diagnosis.

The plain X- ray may not show much. The standard and the golden investigation is MRI. CT scan and Myelogram is also used in certain cases.
